Sertex Connect Survey Privacy Policy

1. You are invited to participate in the Sertex Connect Survey (the “Survey”) conducted by Sertex Broadband Solutions d/b/a Sertex of 22 Center Parkway Plainfield, CT 06374.
2. The Survey which is an opportunity for you to provide input regarding your interest in and preferences concerning high-speed broadband internet in your home.
3. The Survey will take approximately 10 minutes of your time.
4. You will receive no direct benefit from participating in the Survey. However, an indirect benefit of participating in the Survey is your city/town will be able to better assess the needs of your community regarding high-speed internet service. What Sertex learns from you and others in your community may help your city/town make informed decisions whether and to what extent to invest in the construction of fiber broadband network in the future.
5. Participation in the Survey is entirely voluntary. There is no cost to participate, and there will be no negative consequences if you choose not to complete the Survey or any particular item on the survey. There are no foreseeable risks involved in participating in the Survey other than those encountered in day-to-day life.
6. If you are under the age of 18, do NOT participate in the Survey.
7. By participating in the Survey, you consent to Sertex’s collection, use, disclosure, and protection of information about you as described in this Privacy Policy (the “Policy”). If you do not agree to the Policy, please do NOT participate in the Survey.
8. By participating in the Survey, you consent to the following:
   1. Sertex will collect, use, store, and share any information that we collect from or about you in accordance with this Policy.
   2. Sertex will collect information from you directly. The information that you provide to Sertex directly is identified in the Survey’s form multiple choice responses and in the places in the Survey where you voluntarily respond to questions.
   3. Sertex may collect information from you automatically. The information that you may provide to Sertex automatically is the log file reported by your browser when you access the Survey. Sertex’s servers may automatically record certain log file information from you. These server logs may include, but is not limited to, information such as your web request, Internet Protocol (“IP”) address, browser type, browser language, operating system, platform type, the state or country from which you accessed the Service, software and hardware attributes (including Device ID), referring / exit pages and URLs, number of clicks and how you interact the Survey, the pages viewed, and the order of those pages. When you access the Survey, Sertex may also automatically collect general location information (e.g., IP address, city/state and or zip code associated with an IP address) from your computer or mobile device. Please note that if you disable such features, you may not be able to access or receive some or all of the Survey’s content.
   4. Sertex may share some or all of the information that Sertex collects from you with your city/town officials or any other authorities who may fund a capital project to construct a fiber broadband network in your city/town.
   5. Sertex may use the information Sertex collects from you to communicate directly with you. We may send you emails containing information regarding updates or further information about broadband networks. If you do not want to receive such email messages, you have the option to opt out or change your preferences.
   6. Sertex may use some or all of the information Sertex collects from you for research and analytical purposes internal to Sertex, for example, to identify trends and effectiveness of these Sertex Connect Survey campaigns.
   7. Sertex may share your information where required to do so by law or subpoena or if we reasonably believe that such action is necessary to (a) comply with the law and the reasonable requests of law enforcement; (b) enforce our Privacy Policy; and/or (c) exercise or protect the rights, property, or personal safety of Sertex or others.
   8. Sertex will not share any of your information with any others for any other commercial or non-commercial purpose.
   9. Sertex will take reasonable, commercially acceptable measures to protect the information collected from you against accidental or unlawful destruction or accidental loss, alteration, unauthorized disclosure or access. These measures will depend on the sensitivity of the information Sertex collected from you. However, there is no method of electronic storage that is absolutely secure. Therefore, Sertex cannot guarantee its absolute security. Sertex is not responsible for the security measures of any city/town or other third-party authorities with whom Sertex shares your information.
   10. Sertex does not knowingly collect or solicit any information from anyone under the age of 18. In the event that Sertex learn that it has inadvertently collected personal information from anyone under the age of 18, Sertex will promptly delete that information. If you believe that Sertex may have collected any information from a child under 16 through the Survey, please contact Sertex at getconnected@sertexbroadband.com.
